A Dock Receipt is issued to people when they hire a shipping company to pick up a package and transfer it from the shipper to the carrier. The shipper or their sale broker/agent will use this information to prepare the bill of lading. It is design and printed with relevant information to ship goods internationally. This is a receipt with multiple parts, printed on carbonless paper, consecutively numbered and customized with the logo and business name. The sales agent is authorized and instructed by the shipping company to receive and transport goods on their behalf. The FMC license number is also placed on the form.
What is printed on the Dock Receipt Form?
At the top of the receipt, there is normally a statement like " SHIPPER IS RESPONSIBLE FOR REVIEWING AND VERIFYING THAT ALL INFORMATION BELOW IS CORRECT". Then below that, you have two sections, one for the shipper and the other for the consignee. This includes an area for name, address, phone number etc. Below that you will have a separate section that captures:
- Shipping Method (Air or Sea)
- Port Of Landing
- Destination
- Delivery Type (port or door)
- Ship date
- Insurance (select whether the goods are insured or not)
- Document number
The largest section is where you will write in the number of packages/barrels, description of items shipped, cubic feet, weight, and total cost. There is also a terms and conditions section that sate the shippers right and information for the customer to adhere to. The name, address, and phone number of the International Agents should also be stated.
