These are Megaforms or extra size business forms used mainly by health organizations like hospitals. The forms can be up to 120" long, giving you the flexibility to print and present information on one compact sheet, folded to a specific size. It can be made in a continuous format or as a unit set.
The General specifications are:
- Colors: 4 color maximum standard or PMS
- Parts or Plys: 8 Parts maximum
- Paper weights: 12# (.0025 caliper) Minimum to 125# (.009 caliper) Maximum
- Hole Punching: File Punching 14", 17" or 22" multiples
- Numbering: Crash numbering on Collator (special repeat numbering available)
- Perfing: Vertical any location up to 10 perforations Horizontal. 7", 8 1/2", 10", 11", 14" or 17" (Between fold perfs available on request)
Specification for Megasnap (wide Unit Sets):
- Form Width: 25 1/2" to 255" (7", 8 1/2" & 11" increments)
- Form Depth: 5" minimum to 17 1/2" Maximum
- Print Depth: 4" min. to 16 1/2" max.
- Fold Length: Multiple-Parts are Z-folded 25 1/2" can be Z-Folded, Gate Folded, or Flat.
These forms can be used to capture a large amount of information. They are used by Hospitals as Trauma Flow Sheet, Nursing Observation notes, Medication sheet, Anesthesia Records etc.
Extra-Long or Wide carbonless forms
Form Width: 5" minimum to 17" maximum or 25 1/2" to 255"
Print Width: 4" Min. to 16 1/2" Max.
Image Repeat: 25 1/2" and 30" to 120" (custom lengths are also available.
Folds: Different fold type like gatefold, tri-fold, z fold etc. are available.
Fold Lengths: 7", 8 1/2", 10", 11", 14", 17" and other custom widths.
Lock-up: 1/2" between the bottom and the top of the form or somewhere within the length of the form.
