The form consists of:

• Pharmacy receipt with drug administration record
• PCA Control Number
• Pharmacy charge ticket - Shingled tickets are used to write down the dose of the drug, and the related patient to whom the drug has been administered.
• The journal is an 8-1/2″ x 11″ sheet to which the pharmacy receipt and the shingled charge tickets are attached.
• The charge code and the amount charged
• Carbonless paper with the last copy being manila card stock
